What to Bring to Your Visit at Arizona Grown, LLC.
In order to ensure that your MMJ certification processing is completed efficiently and in a timely manner, we ask that you bring some specific documentation in addition to the appropriate fee payment (see Fees & Payment Info) to your visit. Please read the details below to determine which documentation types are most relevant for your visit.
All patients must bring the following to their visit:
Acceptable forms include: Arizona Driver’s License or Identification card; Passport booklet
We accept cash (USD), debit or major credit cards ONLY.
(Including renewal patients who are new to AZ Grown)
Medical documents or records NO MORE THAN ONE (1) YEAR OLD that corroborate your need for medical marijuana certification.
Some examples include: hospital visits, x-rays, MRIs, physician referrals, prescription lists, consultations, etc.
Visits to a specialist (e.g., pain management specialist, psychiatrist, physical therapist, etc.)
Visit with your primary care physician.
*Please note that medical documentation must indicate the presence or diagnosis of at least one QUALIFYING CONDITION (for a list of these, click here). Documentation that is unrelated to a qualifying condition or indicaes acute (versus chronic) illness, injury or pain will not be accepted.
**We are able to fax records requests from our office to your physician(s), but this may interfere with prompt processing of your MMJ application. You are also welcome to fill out a records release before your visit and email it to us. Click here to access the form. Please note that you will be charged an additional 10.00 for this service.
Your current MMJ card. We will need your QPID number to complete your renewal MMJ application.
**Please note that MMJ cards expired for THIRTY (30) days or more prior to the date of your visit to AZ Grown are not eligible for discounted pricing.
